The patients are our #1 concern here at Lima Memorial Health
System. Our Social Workers work through a large variety of programs
to help patients and their families deal with the stresses and
problems associated with a stay in the hospital, as well as the
necessary plans needed after hospitalization.
Social Workers assist patients and families with emotional
stress (separation, chemical dependency, terminal illness, family
violence and long term illness and rehabilitation) and finding
practical solutions (meals, finances, homemakers, equipment and
transportation).
The first goal of Social Services is to return our patients to
at least the level of independence he/she had prior to admission.
We can help our patients return to their homes by providing
adequate support through post-hospital planning and existing
community agencies, such as PASSPORT and Home Health. The majority
of our patients do go home with a Home Health agency following
their discharge.
Referrals may come to social services by anyone and do not
require a doctor's order. Social workers make assessments of
patients and our trained nurses also make continuous
assessments.
Social Services can also provide information on Advanced
Directives.

We can assist with the completion of these documents prior to
admission on Tuesdays and Thursdays from 10 a.m. to 2 p.m. Please

service is free, as well as all the services provided by our Social
Services Staff.
