Comprehensive Health Center
The LMHS Comprehensive Health Center (CHC) is designed to surround you or a loved one with a network of care providers focused on improving quality of life with a chronic disease. In the CHC, we provide the same level of care coordination and support in the outpatient setting as we do in the inpatient setting for high-risk patients.
We are here to ensure you receive comprehensive and coordinated care among your providers that helps prevent or minimize the effects of your condition; this can include comprehensive screenings, regular check-ups, ongoing monitoring and collaborative treatment and education. Currently, the LMHS Comprehensive Health Center encompasses our Diabetic Center, Congestive Heart Failure Clinic, Anticoagulation Clinic and Medical Nutrition Services.
