Lima Memorial Home Health Care works with patients, families, caregivers and physicians to help people maintain their independence at home after being discharged from the hospital or a long-term care facility. Our staff will be there to provide the medical services needed while teaching patients how to continue their own treatments when the time is right. All treatments are under the direction of an ordering physician. Caregivers and the patient are included in the development of the plan of care as well.

Preparation for home care begins long before the staff arrives at a patient’s home. Our staff works with the physicians, other healthcare providers and insurances to coordinate the aspects needed for in-home services.
