When you arrive at Lima Memorial, park by the West entrance. Take the elevator up to the first floor Welcome Center. Check in with your paperwork at the welcome desk. You will be prompted to have a seat around the corner for registration. Once registered, you can make your way to the Outpatient Diagnostic & Imaging Center or to the Outpatient Surgery check in.

Lima Memorial Health System’s (LMHS) physician offices are provider-based clinics. Receiving care from our providers at our clinics will result in a professional or physician charge and a facility charge for services and / or procedures. This will be reflected on statements you receive for provided services.

Thanks to Lima Memorial eHealth, it's never been easier to manage your health and well-being. This secure online tool lets those who have been a patient of Lima Memorial Hospital, or are patients of Lima Memorial Physicians, conveniently access their personal health information.
To activate your account, provide the hospital registrar or your physician's office with a personal email address.

Enter through the Emergency Department and the front desk will activate the elevator for you to go directly up to the PCU unit. Check in at the PCU desk.

Financial Services is located in the Patient Services Building. There is a visible sign on the building. Park in the lot in the back of the hospital. DO NOT enter through the Welcome Center, that is the wrong location.
